This is where you will gather and submit forms including employment verification, criminal background checks, financial statements, reference letters and more. - Application: The application needs to be collected first and includes several forms offering consent and agreements that you will have to submit along personal information.
- Self Study: There will be sections that you fill out together as parents and parts that are filled out as individuals.
- Reference Letters: These are important as they need to be gathered from four non-family members and one family member. Five letters in total and if you currently or previously worked with children, one of the letters must be submitted from your employer at that job.
- Criminal Background Checks: These must be completed by anyone over the age of 12 who lives in the home.
