How does this work?
Our goal is to make this process easy!

Once we receive your initial application we will let you know and discuss meeting at your home.

In the meantime, you will be working on the rest of your paperwork.

We have a simple three step plan for you to follow to get your home study started.  

Step 1

Download the Welcome, Instructions and Checklist

This is very helpful as it instructs you as to how to complete your paperwork!
As well, there are a few additional documents that you will need to provide to us so we can complete your study. 
Make sure to download and print this first! Keep it handy as you get started.

Step 2

There are several forms for you to complete.  Please see below.

(5 total – 4 friends/1 relative)

Pediatrician Report (if applicable)
Insurance and Guardianship

*Florida Adoptive Home Application – this is only required if your adoption takes you outside of the State of Florida.

Step 3

We need photo copies of these important documents.

Driver’s License
Birth Certificate or Passport
Marriage License (if applicable)
Divorce Decree (if applicable)
Previous Year Tax Form 1040
Pet Vaccination Records (if applicable)
Certificate of Naturalization (if applicable)
Military Discharge (if applicable)

REMINDER:  A home study is valid for one year.  An update of your home study is required if your initial study was completed 12 months prior and a child has not yet been placed in your home for the purpose of adoption.
