Some forms are completed electronically while others will be hard copies.

• Welcome, Instructions & Checklist (SUPER helpful)

• Adoption Application -electronic form

• Support Documents -please complete once application submitted

• Inventory (word doc) or Inventory (pdf) -use whichever format you prefer

• Financial Statement -electronic form           

• Insurance & Guardianship - electronic form

• References (3 friends & 2 relatives) - electronic form

• Employment Verification     

• Medical Report

• Pediatrician Report

• Affidavit of Good Moral Character

• Required Background Checks - electronic form

• Driver’s License

• Health Insurance Card

• Birth Certificate or Passport

• First Page of Prior Year Tax Form 1040

• Marriage License (if applicable)

• Divorce Decree (if applicable)

• Current Pet Vaccinations (if applicable)

• Naturalization Certificate (if applicable)

• Military Discharge (if applicable)

You can upload your completed paperwork to your Google Drive folder that we will send you access to. Or if you prefer, we are happy to collect your documents when we visit with you in your home.
